Warning - this post is speculation.
To understand AGP we need to look at how sexuality develops. Gay men are a sort of hybrid of male and female sexuality, which leads me to believe there are multiple aspects to human sexuality. The first is the most base and primal aspect. This is why there is very little homosexuality outside of the human species, unless the homosexuality is merely incidental.
The second aspect of human sexuality, which is uniquely human, relates to gender. A masculinization or feminization of the brain creates a set of interests and personality traits that young children are interested in. Children create a sense of their own gender and the other gender. A typical child might notice that men have breads, a certain facial shape, a certain boy shape, and play a certain role in society. They create a mental idea of what a man is, which differs person to person. A child might develop the idea that a man has short hair, works with machines, and is muscular. If the child identifies as belonging to this gender, he or she will strive to be like it and not find its traits arousing. A child would also create a mental idea of what a woman is. Female traits might include long hair, a feminine face and body, female clothing, and having a male partner. There would likely be some variance as to what constitutes one gender or the other depending on the individual, his or her culture, and the people he or she interacts with at a long age. If the child does not identify as this gender, he or she will find this gender arousing but not have a desire to take on the aspects of this gender, and might even be repulsed by the idea of taking on aspects of this gender.
The child, who is naturally very interested in gender, is doing two different things. He or she is creating his or her idea of what it means to be a man or a woman, and is applying these ideas to the concept of the self and the other. With those who have AGP, the sense of self and other is not yet developed by the time there is an interest in gender. A proper sense of other is understanding that other people exist, are real, having feelings, and need to be cared for. It's a feeling rather than a logical understand. Because of the lack of sense of the other, gender identity develops normally, but the concept of the female does not get applied to an external entity.
The variation in the concept of the female is expressed in the interest of the man with AGP. It's usually a combination of things, with some finding a certain type of clothing important, while others might find long hair or painted nails important. Some find having a vagina important while others do not, possibly because they were unaware of the difference in genitalia during the time they were exploring gender and creating gender models.
Most AGP children eventually develop a proper concept of self and other later in development, but their sexuality is already hard-coded. They are stuck with a lifelong AGP fetish. The more primal sexuality is still present, and that drive pushes them towards seeking a mate and having intercourse. Their sense of self and other are formed, and as long as they continue to have a healthy sense of self and other, they can function about as well as the average man who happens to have a fetish.
AGP can get out of control when the sense of self and other begins to break down or simply never develops. Some men with AGP will have periods in their life with no desire to transition male to female, and other times when there is an intense desire. Another sign of not being able to comprehend the other might be libertarian political beliefs or the desire for a bloody political revolution.
HOW TO TREAT AGP
AGP as a fetish cannot be cured. It developed at a critical time in one's development and is likely permanent, although one might be able to not be exclusively AGP and might be able to reach orgasm from more vanilla fantasies on occasion. In terms of sex, an AGP can rely on his more primal sexual desire and can likely function adequately in the bedroom. Doggy-style sex might be the most conducive to focusing on the primal sexual aspect, and might provide the AGP a method of reaching orgasm without an AGP fatasy.
The aspect of AGP that can likely be treated is the desire to transition. A person can chemically enhance his sense of empathy, which can help him become interested in others and no longer so focused on the self. MDMA is likely the best drug for this purpose, but LSD would likely work as well. I suspect that many men with AGP who feel a sense of gender dysphoria and a longing to transition can lose this longing by taking MDMA or LSD.
